Join our regular Health and Care Forum for people in Huntingdonshire
We meet every two months to talk about different topics. And invite local health and care decision makers along to answer your questions.
Support for people leaving hospital
At this meeting, we will be looking at support for people leaving hospital – with a talk from Cambridgeshire County Council.
The Council will be updating us on what they are doing to put proper support in place for people to keep them safe when they leave hospital. And help them stay independent in their own homes for longer, thus reducing re-admissions. The Council wants to hear your ideas and experiences for improving care.
Integrated Care System for Cambridgeshire and Peterborough
We’ll also be hearing how health and care across our area is being joined up in a new Integrated Care System (ICS) and what the benefits are for local people.
This is a new way to plan health and care services so they can work better together, so the growing numbers of older people and those living with long-term conditions can be supported.
Cambridgeshire and Peterborough Clinical Commissioning Group – who currently plan and pay for most local health services – will be telling us about the changes, answering your questions and listening to your ideas for how this will happen locally.
